What to bring and how to update your records/ governemnt documents from your old civil status, Single to Married, from your old surname to your new surname which is your husband's surname.

  • Bank Records
- Requirements:  Marriage Certificate, Valid IDs
  1.  Bring your Marriage Certificate or a copy of your marriage contract
  2.  Bring Valid ID's
  3.  approach the teller and tell them that you're going to update your marriage status and your family name (for women).


  • SSS
- Requirements: Marriage Certificate photocopy and original, Valid IDs, Member’s Data Amendment Form (E4) form
  1. Bring your Marriage Certificate. (in my case, i brought the original and a photocopy of it but the personnel didn't check the original copy of my certificate, but just to be sure, bring the original copy as well)
  2. Fill up Member’s Data Amendment Form (E4) form. (in my case i printed out one and filled it up in advance to save time. but you could ask an E4 form in the information desk in SSS)
  3. Ask the information desk, what counter/perosnnel will you approach after filling up the E4 form
  4. Submit the E4 form, Marriage Certificate to the personnel assigned and they will update your data.
  5. *but if your employed you can tell your HR that you wish to update your SSS data.


  • Philhealth
- Requirements: Marriage Certificate photocopy and original, Philhealth ID/number, PhilHealth Member Registration Form (PMRF), you could download this on google or you could ask for a form in the philhealth office
  1. ask the guard or any personel where to get your number for updating your civil status.
  2. they will give you a PhilHealth Member Registration Form (PMRF) for you to fill up and it would save you time if you brought one and filled it up in advance.
  3. once your number is being flash/called by the personell, hand them your PMRF, Philhealth ID and Marriage Certificate
  4. After they update your information, they will give you a new Members Data Record (MDR) with your new civil status, Married.
  5. This can be done on Philhealth office near you.


  • PAG-IBIG
- Requirements: Marriage Certificate photocopy and original, Pag-ibig number/HDMF number, Member’s Change of Information (MCIF) you could downlod this form online or you could ask a form in the pag-ibig office, Valid Id,
  1. ask the guard or any personel where to get your number for updating your civil status.
  2. they will give you a Member’s Change of Information (MCIF) for you to fill up and it would save you time if you brought one and filled it up in advance.
  3. once your number is being flash/called by the personel, hand them your Member’s Change of Information (MCIF) and marriage contract.
  4. After they update your information, they will give you a new Member’s change of Information (MCIF) which is updated from single to being married.
